Transcript Request Form

Requests for transcripts must be received in writing. To request an official or non-official copy of your transcript, please complete the Transcript Request Form and return to the Our Lady of Lourdes School of Nursing along with your check or money order, if applicable.

A student's first transcript request is free of charge. Additional requests are $5.00 per request.

Transcript requests should be mailed to Our Lady of Lourdes School of Nursing, 1600 Haddon Avenue, Camden, NJ 08103

Although transcripts can be picked up in person, requests cannot be made in person. The School will contact you when the transcript is ready.
