Health Requirements
Prior to entering NURSING 1 and HEALTH ASSESSMENT, students must have a complete health exam. Students must submit all items listed below under The Cumulative Health Record to castlebranch.com, a secure platform used by the School of Nursing to track a student's health record and immunization status online. Please be aware that the School of Nursing cannot ensure a latex-free environment.
The Cumulative Health Record includes:
- Health history.
- Physical examination.
- Tetanus booster (last date received).
- Hepatitis B immunization Series
- PPD (Completed no earlier than 3 months prior to start day)
- Chest X-ray (only if positive PPD).
- Lab studies (Must be done in same calendar year as deadline date for submission of required documents):
- Flu Vaccine
- Covid Vaccine
- Titers for Measles, Mumps and Rubella (MMR)
- Titer for Varicella (chicken pox)
- Titer for Hepatitis B (series completed)
IF ANY TITERS READ NON-IMMUNE, YOU MUST RECEIVE BOOSTERS. IF THE HEPATITIS B TITER READS NON-IMMUNE, THE SERIES MUST BE RESTARTED. - Complete Blood Count (CBC)
- Comprehensive Metabolic Panel (Chemistry)
- Urinalysis (UA)
**Copies of all lab results must be included with the health record. **
